STEP #1 – FEES  

•  PAY registration fees online and print receipt through MyDsd.
•  If paying by check or cash at the school, Print Fee Form and return with payment to the main office. 
• To ensure that no student is denied the opportunity to participate in programs, courses, or activities because of the inability to pay you can find information on FEE WAIVERS and qualifications here.
• SCHOOL LUNCHES WILL NOT BE FREE THIS SCHOOL YEAR.
•  Apply for Free/Reduced Lunch (English) (Spanish)
• Paper form for free/reduced lunch available in the office.

STEP #2 – POLICY REVIEW & APPROVAL (required by parent and student)

• Log into MyDSD: Click on the Home icon -> Tools -> Sign Documents
• Opt-out form (Optional): Sign and bring in the office if you DO NOT want your student photographed or recorded on video on the rare occasion it may be used by the district or school or outside entity, including the media. English Spanish

REGISTRATION CHECKLIST:  

• Please complete by August 10th to avoid $10 late fee
• Pay fees on mydsd or in office, or fee waiver application filled out and turned into the office with proof of income. 
• Please turn in up-to-date Immunization Form to the NDJH main office (7th graders). For your student's immunization information please contact their physician or the Davis County Health Department.
• Review district policies on mydsd and sign online.
